A: Difficult one! There are 2 moods in which these phrases might be asked. One is the casual questioning mood and the other would be accusative (angry...accusing: you did something wrong!).

In the first case (questioning), I think the difference is that "has it ever occurred" is more open ended...leaving open the possibility that maybe it has not ever occurred. "did it ever occur" is a little more sharp, indicating that it probably should have occurred to you, but didn't for some reason.

In the second case (accusative), both are definitely sharp. "Has it ever occurred to you that I might need to sleep too?!" "Did it ever occur to you to check your watch every now and then?!" Both are angry and imply the other person is not doing something that they should have. But again in "has it ever occurred" leaves open a little possibility that maybe it actually didn't occur to them...even if that is hard to believe.
