In that case they would say "I had a severe headache." while if someone was asking them casually, they will probably say something like "I have a huge headache" instead. the only time I can think where it sounds more appropriate in the present tense is if someone is describing their symptoms to their doctor, and in that case "I have a severe headache" is much better.
