保证 is more like “promise”, sometime you don't have to do anything to promise things, it's just show your confidence. Such as ”我保证不会背叛你I promise that I would never betray you.”
确保 would be translate as “make sure”, which means you may have to devote your efforts on it. E.g” 你需要确保这学期数学课不会挂科 you should make sure that you won't fail the math subject this semester”
