you can say both but the second one sounds more natural. Also, "in itself" means specifically the drug alone, if you say it you mean the drug could be harmful in combination with other drugs or certain conditions

Acetaminophen is not harmful in itself unless you take too much or are allergic.
Aspirin is not harmful itself unless you take more than prescribed.
I’d say you’d want to say “in itself” to emphasise the drug or chemical is known for being mis-used or abused.
Xanax is not harmful in itself. The drug is good if you have the conditions it is meant to help. But many people use it when they don’t need to and it can have side effects.
