This phrase is used to correct someone. Example:
"You recently published a book, right?"
"Not really. I submitted my manuscript to the publisher, but I have not been published yet."

@hideki0 people usually use "not really" to show that it is not quite there. for instance, if you dont want ice cream and then your parents ask, "do you want ice cream" , you can say, "not really". It can also be a polite "no" to some people. just remember to always say thank you at the end to make it sound polite.
