A notch is a cut, a dent, or an incision. In this case it is the deep indentation that is clipping up the baby’s lip.
“She threw the hammer at her brother and made notch on the old chest of drawers.”
“The prisoner had drawn a thousand notches on the wall to keep a tally of his days in prison.”
It is often used figuratively. “My opinion of the president dropped a few notches -marks or degrees- after his derogatory comments to the press.”

In the first picture the baby has a single notch in its lip. The second picture the baby has two notches in its lip. Normally it’s a cut or groove that has been made in a surface.
Other times = at other times
An oral cleft can appear sometimes as a notch on a baby’s lip, and at other times it will have a hole instead of a notch...both are considered to be an oral cleft, although they can be different in appearance.
