Depends on how you are using it.
“I have a great-grandmother who Tap dances“
Or
“I have only one great-grandmother still alive”
Are you stating that you only have one great grandmother or something about your great grandmother ?

Both are correct, but “a great grandmother” is less specific. If you mean you ONLY have one great grandmother, you would say I have one. You can also say “I have a great grandmother” though.
For example, if you are responding to these questions:
Do you have any great grandparents?
-Yes, I have one great grandmother.
Or
-Yes, I have a great grandmother and two great grandfathers.
