Grass can refer to the individual plant or smaller patches/pieces of it, but using the word lawn we imagine a larger (usually man made) area covered in grass. Lawn is an area of grass.
You have grass growing in your backyard, or you can have lawn in your backyard. You can also say your yard is covered in grass, which means that there is lawn on your yard.

There are lots of times where grass and lawn can be interchangeable. However, in order for a section of grass to be called a lawn it has to be near a house or commercial property and mowed regularly. If there is a grassy area that is not mowed regularly it would be called a field or a meadow.
Here are some example sentences where you can use grass or lawn: "I mowed the lawn/grass 3 days ago, but it is growing so fast that I will probably have to mow it again tomorrow." , "I need to ask our neighbor how he keeps his lawn/grass so green and thick."
Here are some example sentences where only the word grass or lawn will sound natural: "We scattered grass seed over the bare patches in our lawn." , "I slept in the soft grass while the sun gently warmed my skin" , "My daughter loves to pull handfuls of grass out and throw it into the wind."
