There is no difference between the two as bottom line; both refers to emotion. I am grateful for that is more proper.

"I feel grateful" has more of an impartial connotation, while "I am grateful" is definite and more natural sounding. "I feel grateful" would be used if someone asked about your feelings, but I can't think of a scenario where "I am grateful" couldn't be used in its place.
