Every weekend means exactly that, each and every weekend. But on weekends just means that on Saturdays and Sundays you do different things than you do on weekdays, it is used as a comparison to differentiate between weekdays and weekends.

Every weekend I go grocery shopping.
On weekends, I get to sleep in instead of having to wake up early and rush to get ready for work.
