girly means someone's personality, or an item, is like a girl's or related to females in some way.

girlish means that they act kind of like a girl, but not as much or often.

adding ish makes the perception vague, they are that but not exactly.

Girlish is used less often overall, mostly as an adjective for attributes of the girl, not the girl herself. So you wouldn't say "she's girlish" but you can say "she has a girlish smile" or "I fell for her girlish charms." It's a little old fashioned and so is used in very limited ways.

Girly is used more frequently. You couldn't use it in either of the sentences I used girlish in, but you can describe most people and things as girly if they are feminine and perhaps a little childish. For people and things that are very mature, you can also just call them feminine.
