So "grateful" goes with "you" like "I'd be GRATEFUL if YOU ......"
And can I use the past form for the future event as you wrote in the second sentence?

If you use "if" then you can use the past form for the future event.
You can also be grateful for things other than people, like
"I would be greatful if it rained tomorrow."
