It's usually an impolite way of telling someone they should have better manners.

Often it's used when the other person failed to say “excuse me” when that would have been the polite thing to do.

It's kind of a rude phrase, so choose your time well. (Interesting that it's rude, since it's purpose is to point out someone else's rudeness...)
