@monica8 In this case, it's referring to the retirement community he was living in. 

Institution = the retirement community
Neglect = a failure to look after

They're suggesting that he might have died because the retirement community wasn't doing their job. Perhaps they weren't watching or taking care of him, or perhaps they were too slow to get him medical attention
