'snitches' is a derogatory term for someone who reveals a secret about someone without that person's consent or who has told a group of people or a person about something bad someone has done.
'Stitches' are what doctors use to sew your wounds together after you have been badly hurt.
So the phrase 'snitches get stitches' basically means that people who can't keep secrets will get hurt by the person or people the secret was protecting.
