¿Cuál es la diferencia entre not only A but also B y not only X but Y ?Puedes dar oraciones como ejemplo.

When I was a high school student, 40 years ago, my English teacher taught me the difference is like below. 1) "not only A but also B" : B doesn't include A. For example, " not only in Japan but worldwide". 2) "not only X but Y" : Y includes X. For example, "not only in Japan but also in Korea". But recently, seems like 1) and 2) are used in the same way. Has it changed? Or my teacher was wrong?

As long as I remember, it has always been this:
"Not in Japan but worldwide"
b doesn't include a if you remove the word only.
"Not only in Japan but worldwide."
y Includes x.
if you said "not only in Japan but also in korea," it would include both.
So yes, both are used the same
